Description

"A Gusto en Casa" (Comfortable at home) is a comprehensive, person-centered care program for individuals with dependency, disability, or chronic illness who wish to remain in their own homes. The project offers personalized social, healthcare, and community support to promote personal autonomy, quality of life, and continued residence in their chosen environment, avoiding unnecessary institutionalization and strengthening local care in rural areas.

Goals

  • Enable people with support needs to continue living safely, independently, and with a good quality of life in their own homes.
  • Promote a person-centered care model that respects their preferences, decisions, and life plans.
  • Support families and caregivers through professional services, community resources, and personalized support that facilitates work-life balance and caregiving.
  • Foster social cohesion and the development of inclusive rural communities capable of providing long-term care in people's familiar surroundings.

Activities

  1. Development of personalized support plans through the intervention of case managers and coordinators responsible for designing and coordinating the necessary resources for each individual.
  2. Provision of personal assistance services, home healthcare, advanced telecare, home adaptations, and assistive technology to promote autonomy and safety.
  3. Development of support programs for caregivers, community participation activities, and social support initiatives to promote the inclusion and well-being of participants.
  4. Coordination among social services, the healthcare system, local authorities, and partner organizations to ensure comprehensive and continuous care.
